Have you considered who would provide care for your loved one(s) if something unexpectedly was to happen to you ?
It is very important you think about what would happen in the event of an emergency and put plans in place to ensure your family member who relies on you, is not overlooked or face any unnecessary risks due to an unexpected situation.
You do need to know if replacement care will get sorted out speedily and efficiently. For many this may simply involve contacting a family member, friend or neighbour who is willing to cover in an emergency. Having an emergency plan in place can help ease your worries if you are not able to care for the person you look after at any point in the future.
This is why we at Brent Carers Centre have developed a simple but fully comprehensive emergency plan template (see Emergency Planning Form below). This will help you think about all the issues you need to consider and are also important to you, should you be unable to continue in your caring role in a case of emergency.
Emergency Planning Form
We have prepared an Emergency Planning Form, which you can simply complete by answering the questions posed. Once completed it will have covered all the essential and important information required to meet your own needs and priorities in the event of an emergency.
Once you have completed the plan you should:

- 1) ensure you have the consent of the people named to be contacted in an event of an emergency;

- 2) ensure your emergency contacts have a copy of your plan or know where you keep your copy in your home;

- 3) present your completed plan to one of our Advisors, who will issue you with a Carers Emergency Card; and
-
- 4) on receipt of your Carers Emergency Card fill in your details, and keep it with you at all times.
This Carers Emergency Card is FREE for any carer who is not eligible to register and use the Carers Emergency Support Service operated in Brent. You will need to have completed an Emergency Planning Form to receive the Carers Emergency Card.
